Type 2 Diabetes: Your Daily Blood Sugar Log

Keeping track

It can be hard to know what affects your blood sugar levels. Keeping a log of your blood sugar levels, your meals, medicine, and any activity might make it easier.

Finding patterns

Start by writing everything down. After a few days, review your logs to see if you notice patterns. For example, if your blood sugar was often high a few hours after breakfast, look at what you ate and drank for breakfast.

Make notes when food choices, activity, stress, being sick, or skipping medicine may have affected your blood sugar.

Be sure to bring your daily logs to your next visit with your diabetes care team. Over time, you and your care team will likely find that it helps to track.
