Your Health: Assess Your Substance Use

The following questions are about your use of alcohol and other drugs. The results of this test can help you see if you might have an alcohol or drug use disorder. In the "answer" column to the right of each question, enter "1" for a yes answer, or "0" for a no answer.

Scoring: To find your score, add up all of the 1-point "yes" answers but do not include numbers 1 and 15. My score is ________.

What does your score mean?

  • 0: You probably don't have a problem with alcohol or drugs. But if you think you may have a problem, see your doctor.
  • 1-3: You might have a problem with drugs or alcohol. If you or someone you know is concerned about your substance use, see your doctor or another health professional.
  • 4 or higher: Your drinking or drug use habits may not be safe or healthy for yourself or others. You may be dependent. If you don't seek help, your substance use may cause serious problems. See your doctor for help.

1. Have you used alcohol or other drugs? (Such as wine, beer, hard liquor, pot, coke, heroin or other opioids, uppers, downers, hallucinogens, or inhalants)

4. Have you gone to anyone for help because of your drinking or drug use? (Such as Alcoholics Anonymous, Narcotics Anonymous, Cocaine Anonymous, counselors, or a treatment program)

  • Had blackouts or other periods of memory loss?
  • Injured your head after drinking or using drugs?
  • Had convulsions or delirium tremens (DTs)?
  • Had hepatitis or other liver problems?
  • Felt sick, shaky, or depressed when you stopped?
  • Felt "coke bugs" or a crawling feeling under the skin after you stopped using drugs?
  • Been injured after drinking or using?
  • Used needles to shoot drugs?
