Here is important information about your care and how to contact the health care team at our clinic.

 

Appointments

To make an appointment, please call the office at (614) 293-4448 or (614) 685-1965. If you need to cancel your appointment, call our office at least 24 hours before your appointment time and reschedule.

 

Telephone Calls

Weekdays (Monday-Friday) 8:00 am to 4:30 pm, call (614) 293-4448 during regular office hours. Our office staff will direct your questions and concerns to the phone triage nurse, who will return your call as soon as possible.

 

After Office Hours and Emergency Calls

After 5:00 pm, weekends or holidays, call (614) 293-4448. Leave a detailed message and the answering service will contact the doctor on call.

If you have an emergency, go to the nearest emergency room or call 911.

 

Family Leave or Disability Paperwork

Family Medical Leave Act (FMLA) or disability paperwork requests can be sent to our office by fax, mail, or you can bring the paperwork to our office. Please allow 10 to 14 business days for the paperwork to be completed.

 

About Your Clinic Visit

  • Your appointment will be at the The James Neurosurgery Clinic located on the ground floor of the 300 West 10 th Ave building. Please arrive 30 minutes before your appointment to allow time to register.
  • When you get to the clinic, check in at The James Neurosurgery Registration Desk.
  • Bring your insurance card, advance directives (Health Care Power of Attorney/Living Will), health history and current treatment information.
  • Bring any medical information that has been requested. This includes copies of all reports and a CD of any x-rays or imaging that may have been done at your local hospital.
  • At times, you may experience a longer wait time for your appointment. It may be helpful to bring something to read or another form of quiet entertainment. Please plan ahead for possible delays in getting back to work, for childcare or transportation.
  • Once the health care team is ready to see you, a Patient Care Assistant (PCA) will get you from the waiting area and take you to the clinic.
  • Your height, weight and vital signs will be checked.
  • An RN will ask you questions about your health history and any problems or questions you may have. This information will be shared with the doctor, nurse practitioner, or physician assistant.
  • If you had a CT scan or MRI at The James before your appointment, the doctor will review the scans and talk with you about the results.
  • Before you leave the clinic, a member of the team will review your discharge instructions, also known as the After Visit Summary (AVS). This includes all of your follow up appointments and other instructions.
