Your Gastrointestinal (GI) Clinic Health Care Team (The James)

 

Many people work together to provide your care. Here is a description of the different members of your GI Clinic team. Use this handout to write down their names and phone numbers.

Health Care Team MembersWhat They Do    Names and Numbers
Attending PhysicianDirects your treatment, and coordinates your care with other doctors and team members.       
Nurse Practitioner (NP)A nurse with advanced training who may do procedures and prescribe medicine. 
Primary Nurse (RN)Plans and directs your nursing care. These nurses provide care and teach you how to care for yourself. 
Patient Care Associate (PCA)Helps the nurse provide care. 
Patient Care Resource Manager (PCRM)Helps coordinate any care you may need when you leave the clinic. 
Pharmacist (PharmD)Prepares medicines for you and works with your doctor to manage your chemotherapy dosing and other medicines to make sure they work best for you. 
Clinical Study CoordinatorHelps to coordinate care or monitor research studies. 
Chaplain/ ClergyA chaplain or clergy is available 24 hours, 7 days a week, to offer spiritual care, emotional support, prayer and other religious rituals. 
Registered DietitianHelps with nutrition, learning and food choices. 
Social WorkerCan help you and your family plan your care after your clinic visit, find resources to meet your needs, and give support to help with the changes caused by illness.
